## About the role

The Community Health Worker (CHW) will support members with complex medical and social needs with managing their own health and wellbeing. CHW will assist in motivating behavioral changes in patients to improve health outcomes in members through education, peer support, and the relaying of shared experiences. This is a job focused on San Fernando Valley. Candidates are expected to be comfortable with street outreach in San Fernando Valley and case managing unhoused patients.

## What they're looking for

- High School Diploma or equivalent
- 2+ years of experience with and comfort working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs)
- Minimum 75% travel required - must have a valid driver’s license, automobile insurance and reliable transportation
- Bilingual in English and Spanish strongly preferred
- Ability to work in a dynamic, outdoors environment
- Ability to work independently as well as part of a team
