## About the role

The Behavioral Health Care Manager is a support role addressing the mental health needs of individuals and their families affected by neurodegenerative diseases. Working under clinical oversight, you will provide care coordination and behavioral health support to patients, caregivers, and their loved ones while implementing Synapticure’s Collaborative Mental Health Care (CoCM) program.

## What they're looking for

- Active clinical licensure or license-eligible standing in social work, counseling, nursing, or a related field (e.g., LCSW, LMSW, LPC, LPCA, LMHC, LMFT, RN, BSN, or equivalent
- 0–2 years of clinical case management, care coordination, or patient support experience
- Strong organizational, communication, and interpersonal skills
- Passion for working with vulnerable populations and providing patient-centered care
