About the role
Our Behavioral Health Care Managers are the consistent, trusted advocates at the very center of our care model. By providing targeted behavioral health support that integrates with the patient’s physical healthcare, they serve as the vital bridge that unifies the entire care team—from primary care provider to psychiatric consultant—around a single, compassionate plan. This is a fully remote opportunity supporting one or more of our partner practices, working Monday-Friday 8:30AM - 5PM Hawaii Time.
What they're looking for
- Support the mental health of patients with cancer on an assigned patient caseload
- Provide brief behavioral interventions tailored for the cancer setting
- Screen and assess patients for common mental health and substance abuse disorders
- Facilitate patient engagement and follow-up care
- Provide patient education about common mental health and substance abuse disorders and the available treatment options
- Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications
