About the role
We are hiring an Enrollment Specialist to enroll patients in chronic care management and/or remote patient monitoring programs, in various states, in partnership with cardiology, internal, primary, and family medicine physician groups, and healthcare systems. Successful candidates will bring experience in patient enrollment and call center experience. The Enrollment Specialist supports the Patient Enrollment Department by reaching out to identified patients and enrolling them in Care Management Programs.

- Make outbound calls to patients and provide information about remote patient monitoring and chronic care management programs. Explain program benefits, patient financial responsibility, obtaining consent, processing device shipment, and enrolling in the program(s).
