The Provider Support Services develop relationships with external Housing and Health Opportunities (H2O) providers to build and support the H2O network.
- Serve as the primary point of contact and subject matter expert on the H2O program for contracted providers
- Manage a high volume of provider inquiries that require research, input, and follow-up to ensure accurate resolution and communication
- Documents and maintains records relative to communications with assigned providers to ensure a mutual understanding of expectations, deliverables, and necessary follow-up
- Plans and leads regular on-site and/or virtual meetings, educational seminars, etc. with contracted providers
- Educates H2O providers regarding appropriate H2O program requirements, including best practices in homelessness services delivery
- Collaborates with various departments or designees to develop training and education as required
- Conducts ongoing review of provider performance and engages in program improvement conversations
- Assists in care coordination for complex cases
- Demonstrates ability to plan, organize, prioritize, and work independently and consistently with limited supervision
